I acknowledge by signing this release form that have been given the full opportunity to ask any and all questions might have about obtaining a tattoo from BeautyX.

I acknowledge that all my questions have been answered to my full and total satis- faction specifically acknowledge that I have been advised of the facts and matters set forth below, and I agree as follows:

I am not under the influence of alcohol or drugs.

I do not have acne, freckles, moles, or sunburn in the area to be tattooed that might be agitated by the tattoo process (healing excluded).

I have looked over my design, checked the spelling if applicable, and give my full consent to the application of my tattoo.

I acknowledge that I am not pregnant

I acknowledge that I am free of communicable disease.

I acknowledge that I have truthfully represented to the associates, agents and representatives of I am over eighteen (18) years of age.

that I acknowledge it is not reasonably possible for the associates, agents and representatives of to determine whether I might have an allergic reaction to the dyes, pigments, or processes used in my tattoo and I agree to accept that such risks are possible.

I acknowledge that infection is always possible as a result of obtaining a tattoo particularly in that event that I do not take proper care of my tattoo, and I have been advised of the signs and symptoms of infection that indicate a need to seek medical care.

I acknowledge receipt of written instructions advising me of proper care of my tattoo and recognize the absolute

necessity of following those written instructions. All questions about the body art procedure have been answered to my satisfaction. I acknowledge that variations in color and design may exist between any tattoos as selected by me and as ultimately

applied to my body. I acknowledge that tattooing is a permanent change to my appearance and that no representations have been made tome as to the ability to later change, alter or remove my tattoo.

I acknowledge that the obtaining of my tattoo is my choice alone and consent to the application of the tattoo and to any actions or conduct of the associates, agents or representatives of necessary to perform the tattoo procedure. that are reasonable

I agree to release and forever discharge and forever hold harmless and its associates’ agents officers and shareholders from any and all claims, damages, or legal actions arising from or

connected in any way with my tattoo or the procedures and conduct used to apply my tattoo and any and all tattoos applied by and its associates, agents and representatives in the future. I acknowledge that tattoo inks, dyes and pigments have not been approved by the federal Food and Drug

Administration and the health consequences of using these products are unknown. I acknowledge that there is a chance I might feel lightheaded, dizzy during or after being tattooed I agree to immediately notify the practitioner in the event I feel lightheaded, dizzy and/or faint before, during or after the procedure.

I agree to follow all instructions concerning the care of my tattoo, and that any touch-ups needed because of my own negligence will be done at my own expense.

have been fully informed of the risks of tattooing including but not limited to infection, scarring, difficulties in detecting melanoma, and allergic reactions to tattoo pigment, latex gloves, and antibiotics. Having been informed of the potential risks associated with getting a tattoo I still wish to proceed with tattoo application and I assume any and all risks that may arise from tattooing.

Before Your Appointment

Do not work out the day of the procedure.

Do not tan or have sunburned face.

Do not take Aspirin, Niacin, vitamin E or IBUprofen hours before the procedure.

No alcohol or caffeine (unless you can’t function without caffeine) on the day of the procedure. on the same day, after the appointment, it’s OK to resume.

No waxing or tinting brows 3 days before the procedure

Please note that you will be more sensitive during your menstrual cycle.

After Your Appointment

Everyone’s skin heals differently and most clients develop patchy areas or the appearance of fading after the first treatment. It is important to understand that this is a two part process and the second touch up treatment will complete the procedure and, after healing, you will see the finished result.

Healing time is a standard 7-10 days; brows will appear dark & thick after treatment; after 7 days brows will become 40% lighter and 10-15% thinner.

DAY 1:  30 minutes to 1 hour post procedure, gently cleanse brows with the provided wipes (Golden Brows) to remove the lymph from the skin, pat to dry with a clean paper towel. Repeat every one to two hours (do this 4 times the first day if you can. If it’s nighttime, do it once or twice)

Wash your face in the evening being careful to avoid lathering the forehead and eyebrows.

DAY 2-10:  Dry heal the eyebrows, meaning, do nothing to them at all. No water, no ointment, no products, etc.

DAY 7-14: Exfoliation of the top layer of the brow is normal from day 4-7; new skin forming causes the area to appear lighter at first during this period of time. In this time, your brows may peel, flake and feel itchy. DO NOT SCRATCH OR PICK – it may cause scarring! You may think they even disappeared altogether – please don’t panic (this is due to the new skin forming over the scabbed area). The color will emerge again. Week 4 is where natural exfoliation has happened and the true color emerges.


  • no makeup, lotions/cleansers (on the eyebrows)
  • no heavy or strenuous activity/sweating
  • no exfoliation/scrubbing
  • no picking/scratching
  • no sun/tanning beds
  • no swimming
  • no sauna/steam rooms
  • no hot tubs
  • no dyes or tinting
  • no shaving, waxing, tweezing (on the permanent makeup)
  • no hair removal
  • no sleeping on your face (do your best)
  • no gardening, no contact with animals.

The procedure site towards the hear, elevated body temperature, or purulent drainage from the procedure site (signs and symptoms that indicate the need to seek medical care)


After the procedure, the area treated will be darker and more intense: this is normal. This will last a few days until the skin heals and peels and naturally exfoliates and the true color emerges. In the first week as the tiny scabs form in order to heal, they may look even darker/thicker due to some minor swelling. The 2nd and 3rd week after the procedure, you may think they even disappeared altogether( this is due to the new skin forming over the scabbed area). Week 4 is where natural exfoliation has happened and the true color emerges. Only you may notice the change. It will look like you had your eyebrows tinted that day ( which looks darker initially). Other people may notice ‘something’ is different , but they are unlikely to identify the source of the difference. The brows may also appear thicker than you might like, however, this settles as this minor swelling recedes and the hair stroke will shorten as a result.


Natural Looking Brows does not have control over your body’s healing process (healing is specific to each individual) but do expect that some residual swelling is normal for all procedures. The area may appear uneven, dry, itchy, tender, red & irritated. This is all 100% normal. DO NOT PICK. These symptoms will dissipate each day and vary on an individual basis. Color WILL fade/soften anywhere from 40% to 60%. At the touch up faded areas will be fine tuned. It is also important to realize that you will need a color boost every 1-2 years after the touch-up to maintain it’s fresh natural appearance. You may need to powder and/or pencil even after the healed results. Remember, this is an enhancement to your natural brows not a permanent one.
